Drawing on recent cases of Botox poisoning in Antalya, Aesthetic and Plastic Surgery Specialist Op. Dr. Büşra Gedik Toprak discusses the potential risks of Botox, symptoms that require emergency intervention, how to choose the right specialist, and the complications that can arise from improper procedures.

WHAT SHOULD BE THE PRIMARY MEDICAL SPECIALTY OF THE DOCTOR YOU CONSULT FOR BOTOX TREATMENTS?

When it comes to aesthetic procedures like Botox and dermal fillers, the primary medical specialties to consult are Plastic, Reconstructive, and Aesthetic Surgery, as well as Dermatology. While other physicians who have received the necessary training and are authorized under relevant regulations may also perform these procedures, the key is not merely the ability to administer the treatment; it is essential to have a deep understanding of facial anatomy and the expertise to effectively manage any complications that may arise.

As plastic surgeons, we study facial muscles, nerves, and anatomical structures in great detail throughout our specialized training. Our surgical experience in these areas also provides us with a significant advantage when planning such treatments.

Targeting the wrong muscle, failing to adjust the dosage correctly, or injecting into an improper area can lead to undesirable outcomes. Therefore, one must consider several factors together: the physician’s training and experience, the authorization of the healthcare facility where the procedure is performed, and the reliability of the product being used. WHAT KIND OF DAMAGE CAN AN IMPROPERLY ADMINISTERED BOTOX TREATMENT CAUSE TO THE FACE? IS THIS DAMAGE PERMANENT?

An improperly administered Botox treatment can lead to various complications on the face.

Primary examples include drooping eyelids, asymmetry of the eyebrows, and a condition known as the “Mephisto” (or “Spock”) look, which occurs when the outer portions of the eyebrows are raised excessively.

In some cases—particularly when a more subtle effect was desired—eyebrow movement may become more restricted than intended, leading to a noticeable reduction in facial expressions.

Incorrect application around the mouth can result in issues such as a lopsided mouth, asymmetry when smiling, difficulty smiling, or liquid leaking from the corner of the mouth while drinking.

Are these effects permanent? When licensed botulinum toxin products are used, such localized muscular effects are generally temporary. We typically expect improvement within 3 to 6 months as the effects of the Botox wear off. However, the recovery time can vary from patient to patient.

The situation is different when counterfeit products or substances with unknown compositions are used. In such cases, it is impossible to know the exact ingredients, the dosage, or how the substance will affect the body. The problems caused by these types of products are not limited to aesthetic concerns; they can lead to serious complications, including difficulty swallowing, respiratory distress, and generalized muscle weakness.

In severe cases of botulism, recovery may take weeks or months, and some patients may experience long-term health issues. Therefore, it should not be assumed that every complication arising from Botox will resolve on its own and completely.

In conclusion, Botox should not be viewed as a simple beauty treatment; rather, it should be considered a medical procedure that must be performed using the correct product and an appropriate dosage, by a physician with a thorough understanding of facial anatomy, and within a safe clinical environment.
